Telehealth Visits
Leavitt Family Medicine telehealth brings the right care at the right time wherever you are. You can see and talk with your primary care provider at your convenience. There is no extra fee for this service. If you need injections, labs or testing, we’ll help arrange that for you during the appropriate time to reduce any possible risk.
These visits are scheduled on our website, or through the patient portal healow app or by calling the clinic at 615-826-5664. Copays, coinsurance and deductibles if any will be the same as an in-clinic visit. Please be online and in the appropriate site for your appointment and we will join you as soon as possible.
